The South Carolina Workers’ Compensation Commission has updated four of the Commission’s standard forms.
Form 50-Employee’s Notice of Claim and/or Hearing Request
The Employee must now note whether he/she admits to being at MMI. The form is also more clear on whether the Form 50 is being used for filing a claim or for requesting a hearing.
Form 30-Request for Commission Review of a Single Commissioner’s Order
Mediation status has been added to the form, and the form may now be served electronically (using the email address of the parties listed on eCase).
Form 18-Periodic Report (Filed by the carrier every 6 months)
The Form 18 can no longer be used to request an informal conference or to provide any other note to the Commission. An Informal Conference must be requested by letter or email to the Commission (with an updated Form 18 and MMI report).
Form 32-Employee’s Request to Waive Filing Fee
The word “appeal” was removed from the form’s title because this form can be used by a pro se Claimant for waiver of any filing fee, not just the Form 30 appeal filing fee.
